The crash of an F-16 late last year in California during a routine training flight that prompted the pilot to eject was caused by “misaligned” fan blades, which prompted engine failure. The United States Air Force announced Friday that the service is updating its personnel data systems to align with the Department of Defense’s (DOD) standardized list of Religious Affiliation Codes.
